A scar may be far more than a line on the skin. It can be raised, tethered, painful, itchy, discoloured or tight enough to restrict movement. For patients who have already tried creams, massage or non-specialist treatments without meaningful improvement, complex scar treatment options require a different standard of assessment: one that considers the scar’s biology, its location, the surrounding tissue and the person living with it.

The right treatment may involve advanced laser surgery, carefully planned scar revision, injection treatment or a staged combination. The aim is not to promise that a scar can disappear completely. It is to improve its quality, comfort, function and visual integration with the surrounding skin as safely as possible.
Why some scars need specialist care
A complex scar is not defined only by its size. A small facial scar may be highly visible and psychologically distressing, while a scar across a joint, eyelid, lip, neck or chest can create significant functional difficulty. Burns scars, traumatic scars and scars following major surgery may also behave unpredictably because they involve deeper layers of skin and soft tissue.
Some scars remain red and active for many months. Others become thick, raised or increasingly firm. A tethered scar may pull inward, causing a depression or altering the contour of the skin. Hypertrophic scars stay within the original wound boundary but can be elevated, itchy and uncomfortable. Keloid scars extend beyond it and carry a recognised risk of recurrence, particularly after inappropriate surgery.
Skin tone also matters. In darker skin, inflammation can lead to prolonged changes in pigmentation, including darkening or lightening after an injury or procedure. This does not rule out treatment, but it makes experienced treatment selection, conservative settings and close follow-up especially important.
Assessment before complex scar treatment options
The consultation is where the treatment plan is made or undermined. A consultant-led assessment should establish how the scar developed, whether it is still changing, what treatments have already been attempted and whether symptoms such as pain, itching, sensitivity or restricted movement are present.
The examination considers depth, thickness, colour, vascularity, texture, pliability and the direction of tension across the scar. It also assesses the surrounding skin. A scar that appears broad may be driven by ongoing tension. One that looks sunken may need release beneath the skin rather than surface resurfacing alone.
Timing is equally important. While some mature scars benefit from intervention years after injury, early treatment can be valuable for active red scars, post-operative scars and burns scars. The best approach depends on the individual scar rather than a fixed rule that every scar must be left for a year before treatment begins.
Clinical photographs may be used to document progress, particularly where treatment is staged over several months. This provides an objective record and helps set realistic expectations. Scar improvement is often gradual, and meaningful change may be seen in colour and symptoms before texture improves.
Advanced laser treatment for scars
Laser therapy can be highly effective when chosen for the correct scar characteristic. It is not one treatment, and not every laser is suitable for every scar. Specialist laser practice involves selecting the wavelength, energy, treatment density and timing according to the scar and the patient’s skin type.
Red, itchy and active scars
Vascular laser treatment can target the blood vessels that contribute to redness in an active scar. Reducing this vascular component may also help itching, sensitivity and persistent inflammation. This can be particularly helpful for hypertrophic scars and selected post-surgical or burns scars.
Treatment is usually delivered as a course rather than a single session. The response is assessed over time, with settings adjusted carefully to balance clinical benefit against avoidable irritation or pigment change.
Thick, stiff and contracted scars
Fractional ablative laser treatment creates precise microscopic treatment channels within the scar. This stimulates remodelling while preserving untreated skin between the channels to support healing. In appropriate patients, it can improve thickness, firmness, texture and pliability.
For scars that restrict movement, improving pliability may have a practical effect on daily comfort and function. A contracture that is severe, however, may still require surgical release. Laser treatment can complement surgery, but should not be presented as a substitute where a scar is physically shortening the skin or distorting an important structure.
Uneven texture and acne scarring
Depressed scars, including acne scars, often have multiple components. Laser resurfacing may improve surface irregularity, but a deeply tethered scar may first need release from the tissue beneath it. Depending on the scar pattern, treatment can include subcision, focal surgical techniques, resurfacing or carefully selected filler support.
The most successful plans address the mechanism of the scar rather than applying the same device to every indentation. This is one reason why complex acne scarring is best assessed by a clinician experienced in both reconstructive techniques and laser surgery.
When surgery is the better option
Scar revision surgery may be the most appropriate route when a scar is wide, poorly positioned, tethered, unstable or causing distortion. It can also be necessary after traumatic wounds, skin cancer surgery, burns or previous operations where there is tissue loss or altered anatomy.
Surgery does not erase a scar. It replaces or rearranges an unfavourable scar with one designed to heal in a more discreet direction or under less tension. Techniques may include direct revision, geometric rearrangement, release of contracted tissue, grafting or local flap reconstruction. The procedure selected depends on the location, blood supply, tissue quality and functional requirements.
A key trade-off is that surgery creates a new wound and therefore a new scar. In patients with a history of keloid formation or delayed healing, the decision must be particularly measured. Preventative strategies may include silicone therapy, pressure where appropriate, steroid treatment or early laser management as part of the post-operative plan.
Injections, silicone and supportive treatments
Not every useful treatment involves a laser or an operation. Steroid injections can help soften and flatten selected hypertrophic and keloid scars, although repeated treatment may be needed and over-treatment can thin the skin. Other injectable agents may be considered in specialist settings for resistant scars.
Silicone gel or sheeting can be useful for active raised scars, especially after surgery or burns. It is a low-risk measure but relies on consistent use. Pressure garments have an established role in certain burns pathways, while scar massage and moisturisation may help comfort and mobility once a wound has healed. These measures are supportive, not replacements for assessment where there is contracture, persistent pain or progressive thickening.
A staged plan produces better decisions
Complex scars rarely respond best to a single intervention. A patient may begin with vascular laser treatment to settle redness and itch, proceed to fractional laser treatment for stiffness and texture, then consider limited surgery for a residual tether. Another patient may need surgical release first, followed by laser treatment to optimise the new scar as it heals.
The order matters. Treating a scar too aggressively, too early or with the wrong modality can prolong inflammation and compromise the result. Consultant oversight is especially valuable when scars involve the face, joints, previous burns, darker skin tones, recurrent keloids or a history of unsuccessful treatment elsewhere.
Recovery also varies. Laser treatment may involve temporary redness, swelling, crusting or a period of social downtime, depending on the modality and intensity. Surgical revision requires wound care and a longer maturation period. A specialist discussion should cover likely recovery, possible pigment change, recurrence risk, expected number of sessions and the outcome that is realistically achievable.
A scar deserves careful clinical judgement, particularly when it affects movement, confidence or comfort. The most effective next step is usually not choosing a treatment from a menu, but obtaining a precise diagnosis and a plan that respects the scar’s behaviour, the skin around it and the result that matters most to you.
